Gulf City - Ban Appeal Format



Please copy and paste this template and fill it up in a new thread topic on this board when submitting a complaint. Read the board rules before posting.



1. Your In-Game Name / Discord Tag:
(Example: John_Doe / User#1234)


2. Name of Accused Player or Staff:
(Who are you reporting?)


3. Date & Time of Incident:
(Approximate date and time of what happened)


4. Location / Context of the Incident:
(Where did this happen? E.g., In-game, Discord, Forum)
(Briefly explain the situation or context)


5. Rule(s) Broken or Misconduct Type:
(e.g., RDM, Meta-gaming, Staff abuse, Disrespect, etc.)


6. Detailed Explanation of the Incident:
(Explain what happened. Stick to the facts and remain respectful.)


7. Evidence (Required):
(Links to screenshots, videos, chat logs, etc.)
Upload to services like Imgur, YouTube, or Streamable and paste the link.


8. Witnesses (if any):
(Names or tags of others who saw the incident)
(Optional but helpful)


9. Have You Tried Resolving the Issue Privately?:
(Yes/No – If yes, explain what happened)
This is encouraged before posting, especially for minor disputes.


10. Additional Notes (optional):
(Any extra information you want to add)
